Your Diabetes Care Team

Have you ever wondered who makes up your diabetes care team? It’s a group of healthcare professionals who are there to support you on your journey with type 1, type 2, or gestational diabetes. It’s important to know who’s on your team so you can get the best care.

Key Team Members

Your team typically includes a variety of healthcare professionals. Your team usually begins with your family physician or nurse practitioner. These primary care providers can screen for diabetes, diagnose it, and make important referrals based on your needs.

A nurse is another key member, often providing diabetes education to support your self-management of your diabetes. You may also work with a dietitian, who can help you understand how food affects your diabetes. A dietitian can help you create a meal plan that fits your personal preferences, values, and beliefs about food. A kinesiologist or exercise specialist may also support you in learning how to move more, find exercises that are enjoyable and help to maintain overall good health.  Many nurses, dietitians, and kinesiologists may also be Certified Diabetes Educators (CDEs), indicating that they have additional knowledge and skills related to supporting people living with diabetes.

A clinical pharmacist is another valuable team member. You might find them in a hospital or community setting. They may help you understand your medications, particularly related to the timing of your medications and possible side effects.  Pharmacists are also very helpful at suggesting strategies to ensure medications are taken regularly and ways to minimize side effects.

Mental health is also an important part of diabetes management. Your team may include a social worker, psychologist, or counsellor to support your mental health and well-being. These professionals may be available through your diabetes education program, primary care setting, or require an additional referral.

In some instances, you may be referred to a diabetes specialist called an endocrinologist. This is a specialized doctor with extensive knowledge and training in diabetes, and can work with your team to create the best treatment plan for you.

The Most Important Team Member

The most important member of your diabetes team is you. As a person living with diabetes, you have valuable insight into how diabetes impacts your daily life. You can discuss your care plan with your team and advocate for a plan that best suits your needs. For instance, if you’re not aware that you can see a dietitian, you can ask your doctor for a referral. Learning about the healthcare professionals who help you manage your diabetes is important for building a care team that can support you throughout your journey.
